Referral criteria
To schedule an appointment with Medical Imaging please fax your physician order to (602) 344-1313. For invasive procedures please include current H& P and lab orders.

Reminder calls are made the day prior to your appointment and patient preparation instructions are provided. If you have any questions please contact Scheduling.
