What is MyChart?

MyChart is a secure online portal that allows patients to access their health information 24/7. Patients can access MyChart from any Internet connected computer or smartphone device.

With MyChart you can...

  • Review instructions about your care
  • Receive and review test results
  • Email your care team with non-urgent medical questions
  • Review your medications, immunizations, allergies and health history
  • Request prescription refills from your provider
  • Schedule a visit with your provider

Requirements to Register for a MyChart Account

  1. You must have an existing medical record on file with Maricopa Integrated Health System.
  2. If registration was not done for you during your clinic appointment, you must enter the activation code you received during registration for your inpatient or outpatient services. 
  3. You must enter a valid email address upon registration to receive all MyChart communications.
