New Resident Orientation: Employee Health

Employee Health

You must complete a physical exam at MIHS Employee Health prior to June 13, 2008. Employee Health is located behind the Emergency Department in a single story building (see MIHS Campus Map click here). Hours are Monday through Friday, 7:30am to 3pm. Contact Employee Health at (602) 344-5210. Please complete the enclosed forms, print, and bring them with you to your physical exam. Remember to bring any immunization records you have when you visit Employee Health. Drug testing is performed at the time of your exam. Please note that you will not be allowed to begin your residency until the exam has been completed and test results have been received.

Complete the following forms and bring them with you to your exam.

• Employee Health Registration Form
• Employee Health Assessment Consent Form
• Summary of tests and immunizations form
• OSHA Respirator Questionaire
• TB Respirator Fit Form
• Employee Health Pre-Employment Post Job Offer Form
• Employee Health Latex Screening Form

Some hints for making your EHS visit successful:

• Complete forms online, print, and bring to Employee Health.
• Visit Employee Health by June 13, 2008.
• Bring immunization records with you.
• Allow at least 30 minutes for the exam.
• Schedule an appointment, call 602-344-5210 (this is not mandatory, but is recommended)

NOTE: YOU WILL NOT BE ABLE TO START YOUR RESIDENCY UNTIL YOU HAVE BEEN CLEARED BY EMPLOYEE HEALTH SERVICES.
