New Resident Orientation at the Copa

Getting Started

To get you started, we have provided links to forms and documentation that are required for your residency at the Copa. Please note there are some documents that are time sensitive and must be returned by May 1, 2008. Other documents and forms must be completed and returned prior to Orientation. There are also forms that you must complete and bring with you to Orientation or when you visit Employee Health Services.

You have also been sent, via U.S. Mail, documents that contain sensitive information and/or documents that require an original signature and must be returned prior to Orientation. Those materials include your MIHS email address, Arizona Postgraduate/Permit Training Application, U.S. Medical School Verification, and your MIHS contract.

Further in this section it is noted whether each form is online or has been sent via U.S. mail, and when each form is due to Academic Affairs.

Please familiarize yourself with the items listed below and their due dates. It is imperative that you adhere to the due dates to ensure that everything for your residency is in place and you will be able to start on time.

Resident Information Form

Complete online, print a copy for your files, and submit via email by May 1, 2008. You will be asked to sign your form at Orientation.

Background Check

A link is provided to Universal Background Screening, who will conduct a background check at no cost to you. Complete online process by May 15, 2008 to ensure the results are returned prior to Orientation. Also included for your information is a link to the MIHS background check policy.

Postgraduate Training Permits - Sent via U.S. Mail

Please complete only ONE (for either MD or DO) of the following forms according to your credentialing. There is no cost to you. (NOTE: Podiatry residents do not have to complete either training permit application)

ALLOPATHIC RESIDENTS ONLY: Arizona Medical Board Postgraduate Training Permit Application AND Arizona Statement of Citizenship & Alien Status. Complete, both forms that you received in the mail, in their entirety, and sign. Remember to include with your application a copy of one or more documents as evidence of your citizenship or alien status (a list of acceptable documents is included with the form). Return via U.S. mail to Academic Affairs by May 1, 2008. NOTE: If you do not complete the form in its entirety, you will delay the processing of your training permit. You will not be allowed to start your residency until your training permit has been received.

OSTEOPATHIC RESIDENTS ONLY: Arizona Board of Osteopathic Examiners in Medicine and Surgery Permit Registration. Complete form that you received in the mail in its entirety, and sign. Return via U.S. mail to Academic Affairs by May 1, 2008. NOTE: If you do not complete the form in its entirety you will delay the processing of your training permit. You will not be allowed to start your residency until your training permit has been received.

Resident Physician Employment Contract - Sent via U.S. Mail

Your MIHS contact has been mailed to you, and includes 2 copies of page 1. Fill in your address and phone number under the “Legal Notice” section and sign both copies. Return both copies of page 1 from the contract. Retain pages 2-18 for your files. You will receive a completely signed copy of page 1 at Orientation.

Verification of Medical School Training

U.S. Medical School Verification (FOR U.S. MEDICAL SCHOOL GRADUATES ONLY): Sent via U.S. mail. If you graduated from a U.S. Medical School, a letter of verification has been mailed to you. Please sign and date under the “For Student to Complete” section. Return the letter to Academic Affairs by June 1, 2008. (DO NOT DELIVER DIRECTLY TO YOUR SCHOOL.)

Certification for ECFMG (FOR IMG’s/FMG’s ONLY): If you are an IMG/FMG, you MUST present a valid ECFMG Certificate prior to starting your residency. You can mail your ECFMG prior to Orientation OR bring it with you to Orientation. You will not be able to start your residency without your ECFMG Certification on file with Academic Affairs.

Medical School/Residency Diploma: Please provide a copy of your medical school and previous residency diploma (if applicable). You can mail prior to Orientation OR bring it with you to Orientation.

USMLE / COMLEX Scores

Please provide copies of all USMLE/COMLEX scores, preferably by June 1, 2008, or as soon as you receive them. A link to the MIHS policy, “Passage of USMLE or COMLEX Examinations,” is provided below for your information. This policy does not pertain to Podiatry Residents.

Acknowledgement of Confidentiality

Read carefully, print 2 copies, and sign. Retain one signed copy for your files. Return one signed copy via U.S. mail to Academic Affairs by May 1, 2008.

Loyalty Oath

Read carefully, print 2 copies, and sign. Retain one copy for your files. Return one signed copy via U.S. mail to Academic Affairs by May 1, 2008.

Human Resources

Read carefully, print 2 copies of each form and sign. Retain one copy of each form for your files. Return one signed copy of each form via U.S. mail to Academic Affairs by May 1, 2008.

Lab Coats

Complete online and submit via email by May 1, 2008.

MIHS Email Addresses - Sent via U.S. Mail

Your email address, login and password have been mailed to you, along with instructions for accessing your email from outside of MIHS. Please check your MIHS email account often, as we will be sending valuable NROC 2008 information to you via that email account. After April 11, we will no longer be communicating through your personal email account.

National Patient Identifiers (NPI)

You are not required to have an NPI for your residency at MIHS, although you may need it for outside rotations. We strongly suggest you apply for your NPI now, just in case. You will need to submit this application on your own. Once you receive your NPI number please provide this information to Academic Affairs.
