MIHS, in compliance with Federal and State regulations, requires certain documentation and forms to be completed prior to the beginning of your employment. Please pay close attention to due dates and requirements for each item listed below. You will not be able to begin your residency until all requirements are met, with the exception of the Employee Authorization for Direct Deposit.
Social Security Card
You must bring your Social Security Card to Orientation. You will be paid by the name on your Social Security Card. If you do not have a card or need to change your name, please click below for Social Security Office information. You will NOT be paid without presenting your Social Security Card. Copies are not acceptable.
Employment Eligibility Fom (I-9)
You are required to provide proof that you are eligible to work in the United States. Click below for a list of documents that are acceptable for proof of eligibility. You must bring one item from list A, OR one item from list B and one from list C. You will not be able to begin your Residency without providing proof that you are legally eligible to work in the USA.
Federal Form W-4
Complete the lower portion of the form. If necessary, refer to the worksheet on the reverse side. Mail the completed form back to Academic Affairs by May 1, 2008. You must complete and submit this form to get paid!
Arizona Form A-4
Complete the form. Mail the completed form back to Academic Affairs by May 1, 2008. You must complete and submit this form to get paid!
Employee Authorization for Direct Deposit
Read the Employee Authorization for Direct Deposit information sheet. Complete the form and attach a voided blank check or suitable bank document. If you do not have your checks yet, ask your bank for a form to submit for direct deposit. You can mail prior to orientation or bring it with you to Orientation.
