Arizona Primary Care Program (formerly Tobacco Tax program)

This program provides prenatal care, primary care and preventive dental services at 10 Family Health Centers located in the communities such as Avondale, El Mirage, Chandler, Guadalupe and Central Phoenix.

Eligibility Requirements
  • Be a resident of Arizona
  • Be ineligible for AHCCCS, KidsCare and Medicare and otherwise without insurance for primary care
  • Reside in a household with gross annual income less than 200% of Federal Poverty Level (FPL) Guidelines
Eligibility Period

Patients are eligible for a period of 12 months following their enrollment date.

  • Prenatal care services
  • Specialty diagnostic services ordered by Primary Care Physician
  • Laboratory, radiology and pharmaceutical services
  • Preventive dental services and physician services
Non-covered Services
  • Specialty treatments
  • Inpatient services
  • Emergency, urgent care and home health care services
  • Outpatient surgical, physical therapy and psychiatric services
